What is the Sahara Health Promotion and Prevention Program?

The Sahara Health Promotion and Prevention Program (SHPP) is a Locally Driven Population Health Model (LDPHM) funded by Ontario Health. This initiative is designed to address the unique health needs of diverse communities in the Region of Peel. The program aims to promote holistic well-being by implementing culturally and linguistically tailored health interventions and preventive care strategies that resonate with the community's values and practices
The SHPP builds strong partnerships with local stakeholders, including healthcare providers, community organizations, public health departments, places of worship, and educational institutions, to ensure collaborative health promotion efforts. The program focuses on enhancing health outcomes through active community engagement, evidence-informed decision making (EIDM), and strategic preventive care planning and goal setting.

Pillars of SHPP

The Sahara Health Promotion and Prevention Program (SHPP) is built on several essential pillars: preventive planning encompassing breast cancer screening and education, diabetes education, and support for mental health and addictions. Additionally, the program emphasizes primary care attachment and connecting participants to wraparound supports. These pillars are tailored to address the diverse cultural needs of the Peel community, ensuring that health interventions are both culturally sensitive and relevant.

Prevention Program

  • Breast Cancer Screening & Education
  • Learning about signs and symptoms of breast cancer
  • Understanding risk factors
  • Information on where to get screened
  • How to perform a self-examination

Diabetes Education

  • Community-based Diabetes Education Program
  • Access to seminars covering various diabetes-related topics
  • Personalized diabetes management plans and support

Mental Health & Addiction Supports

  • Education about Mental Health & Addictions
  • Access to individual assessments and care plans
  • Workshops available for Mental Health & Addictions support

Primary Care Attachment

  • Assist participants in finding and attaching to a primary care provider or family doctor
  • Follow-up with participants to assess progress in connecting with a family doctor

Connection to Wraparound Supports

  • Help navigating resources available within the community
  • Access to culturally and linguistically appropriate services

Requirements for Program

  • Resident of Region of Peel (Brampton, Caledon & Mississauga)
  • Anyone 18 or older, regardless of status, can access these services
