The Sahara Health Promotion and Prevention Program (SHPP) is a Locally Driven Population Health Model (LDPHM) funded by Ontario Health. This initiative is designed to address the unique health needs of diverse communities in the Region of Peel. The program aims to promote holistic well-being by implementing culturally and linguistically tailored health interventions and preventive care strategies that resonate with the community's values and practices

The program focuses on enhancing health outcomes through active community engagement, evidence-informed decision making (EIDM), and strategic preventive care planning and goal setting.
